Online Survey - A ‘draft’ Model of Care for north Powys

Closed 19 Jan 2020

Opened 17 Dec 2019

Overview

During the summer of 2019, we came out to meet with residents and professionals across North Powys to learn more about what keeps you or your family safe and well in your home and community, your wider area, and out of county health and care services today. In addition, we also asked you what health and wellbeing services you would like to see more of in the future.

The experiences and valuable insights you have shared showed just how diverse and unique our communities are in relation to our own wellbeing today, and what we would like to see change. The comments we received have contributed to our ‘draft’ Model of Care for north Powys, and we hope that your experiences have been reflected throughout the new model, and demonstrate our commitment of working together to transform the way we deliver health and wellbeing services in the future.

Below are a series of questions asking as to what extent you agree or disagree with the ‘draft’ Model of Care, and a final question for any additional information you would like to share.
